When I use the agent mode with GPT-5.1 or Claude, they can provide the analysis process and modification records. However, in some cases, the code might not have been modified, meaning the conclusion does not match the actual result. Why is that?

I used the agent to perform voiceover adaptation for the current file, as shown in the screenshot: the analysis process, as well as the generated "changes.log.md" file. However, the actual code changes only involved modifying three lines of code (as shown in the figure), which does not match the analysis process.
